CRITERIA FOR CHRONIC PAIN MANAGEMENT PROGRAM

Patients who may benefit from participation in a chronic pain program are those who have not responded to physical therapy and/or mental health treatment within a reasonable amount of time (minimum 6 months post-injury/illness) or exhibits pain behavior, functional limitations and/or mental or emotional dysfunction that is disruptive to activities of daily living. Participation may also be of value if the patient is facing significant, permanent loss of function that will require major readjustments, has pain that persisted beyond the expected tissue healing time or whose diagnostic finding are insufficient to explain the pain. The patient who continues to display unrealistic expectations regarding pain relief and treatment outcome or whose physical or mental impairment is greater than previously thought should be referred to such a program early in treatment to prevent the development of an excessively disabled lifestyle role. Patients with adverse interpersonal relationships or a documented history of chemical dependency/abuse or excessive use of healthcare services would be candidates for eligibility evaluations as well. Patients usually require a physical and psychological evaluation to determine if admission to a chronic pain management program is appropriate.


INITIAL PRESCREENING

This questionnaire is usually completed during the Functional Capacity Evaluation (FCE) within 15-30 minutes. It provides information about the patient’s view of the rehabilitation process and their perception of the likelihood of recovery. It helps to determine whether the patient is focused on pain, how they deal with it, and if they are emotionally distressed (depressed and/or anxious). This information is the basis for the program staff to design a specialized, effective treatment plan for each patient, and to help the psychology staff identify patients that are most likely to benefit from psychological service in conjunction with their physical treatment.

This assessment will help establish baseline capabilities to assist in establishing work hardening programs, work conditioning programs and/or return to work parameters.

Functional capacity evaluations provide the participant’s referral source with the data necessary to determine the current physical work demand level and to identify any limitations. The information will help the therapist and the referral source in determining a time frame for the participant’s safe return to work and will establish safe parameters to prevent re-injury.

Northeast Rehab Center Proudly Announces the Addition of an Interdisciplinary Chronic Pain Management Program

Northeast Rehab is proud to announce a new addition to its list of patient services: an Interdisciplinary Chronic Pain Management Program. Our treatment team consists of a Medical Director, Psychologist, Licensed Physical Therapists, Nurse Case Manager, Biofeedback Technicians and Massage Therapists. We are able to meet a variety of pain patient needs because we offer a range of diagnostic, prescriptive, and interventional services.

"Pain" is the largest reason for initial contact with any physician for the vast majority of medical problems, e.g. abdominal pain, chest pain, limb pain, low back pain, etc. Although some of these problems are acute (short-term) in nature, chronic pain syndromes are complex and last longer than six months. Symptoms can include: ongoing severe physical pain, progressive physical disability, progressive inability to perform everyday tasks, increased depression and/or anxiety, increased irritability, and increased feelings of hopelessness, helplessness and worthlessness.

Because chronic pain syndromes are so complex in nature, multiple specialties are needed in order to provide accurate diagnosis, treatment planning, and intervention. Northeast Rehab has combined all of these disciplines (medical, physical, and behavioral) in order to best serve the patients needs: We believe that no case is too long-standing or too difficult to help. We work with chronic back pain, neuropathic pain, joint pain, head injuries, Chronic Fatigue Syndrome, Fibromyalgia, and Reflex Sympathetic Dystrophy, among others.
